Our team of home care professionals work with the physician, the patient and the family to develop a custom-tailored, complete and cost-effective treatment plan. Home Health Care is delivered by a team of specialized, compassionate individuals who work together to help each patient return to maximum function, including:

 

Physical therapy 

Physical therapy helps patients maintain, develop and restore physical function that may be lost due to injury, disease or other causes. Physical therapy involves treating pain, range-of-motion, strength, posture, balance, endurance, control and sensation.

  • Continuation of physical therapy after surgery or illness
  • Exercises for strengthening to improve endurance
  • Bed mobility
  • Walk/movement training
  • Teaching the use of walkers, canes and crutches
  • Fall risk assessment and prevention
  • Pulmonary and cardiac rehabilitation 

Occupational therapy 

Our hospital’s  occupational therapy team trains patients to take care of themselves when a health problem limits normal, day-to-day activities, such as bathing, dressing, eating, preparing meals, doing housework and managing personal business. This type of therapy can help patients become more active and willing to care for themselves. Therapy includes:

  • Exercises related to upper extremity activities and function
  • Measures to improve self-care, dressing and activities of daily living
  • Assessment for visual and spatial problems
  • Energy conservation measures to improve endurance
  • Improvements for home safety and increased independence

Speech therapy 

Speech therapy helps patients improve their ability to communicate. This includes speech, which is how sounds are made, and language, which involves understanding and choosing the correct words. A speech therapist can also help patients with swallowing problems, as well as patients with memory loss.

Skilled nursing 

Registered nurses (RN) evaluate patients and develop a plan of care under the direction of the patient’s physician. The main goal for nursing is to help patients get back to their previous level of functioning. Nursing services include:

  • Complete nursing assessment
  • Specialized patient teaching
  • Medication teaching 
  • Wound care teaching and management
  • Urinary catheter care
  • Diabetic teaching and management
  • Colostomy teaching and management
  • Pain control 

Home health aide 

The home health aide helps the activities of daily living, such as: 

  • Bathing - bed, bath or shower
  • Skin and nail care
  • Range-of-motion exercise
  • Assisting with transfers and walking/moving 

Medical social services 

Our hospital’s social services specialists assist patients with problems arising through illness or everyday activities. We perform psychosocial assessments, counseling and, education. Social services can also put you in touch with, community resources and help with long-term planning. These services provide:

  • Emotional support to cope with problems relating to the patient’s illness
  • Information on community resources, financial assistance, fiduciary services and power of attorney services
  • Financial consultations including application for financial aide
  • Assistance with identifying long-term care needs and placement, if necessary
